Eligibility
is generally: Adult applicants who have a psychiatric or co-occurring
psychiatric and substance use disorder who are homeless or being
discharged from a hospital or local mental health authority with no
identified housing at the time of discharge. They also have to be
involved with any DMHAS funded provider.
